A complex RCT case by Dr. Adrian Jones
S-shaped curves in molar canals are ideal for putting the VS Flexi Blue files to the test. These files offer excellent flexibility and allow for conservative shaping of the canals, thereby preserving precious tooth structure.
First up: a UL6. Periodontal treatment has been completed and stabilised. This was a necrotic case with a history of an acute flare-up a few months ago. I used the existing filling as a reference to help orient the access cavity. Numerous pulp stones were encountered.
I prepared the DB and palatal roots first, allowing the MB root to soak in disinfection solution in the meantime. I instrumented the MB root system up to size 20/06, and up to 25/06 in the palatal and DB roots. Irrigation was performed using full-strength sodium hypochlorite and Endo solution, delivered with Irriflex tips. I used a yellow EndoActivator for agitation and micro-suction to dry the canals.
Total Fill sealer was placed and carried to working length using a size 25/029 gutta-percha point. I customised a size 20/029 GP point for the MB root, a 25/029 GP point for the DB root using a taper plate, and used a master fit GP point for the palatal root. The tooth was then sealed with LuxaCore.
Each tooth is as unique as a fingerprint, so a flexible, hybrid approach was taken to cleaning and preparing the canals prior to obturation.
